For an Adult Card (14 and older):

Complete and sign an application and present:

*Valid ID with photo, name and CHICAGO address( driver’s license, IL State ID, etc.) OR

*2 pieces of current identification. One must include name and address (personal check; current utility bill; mail with current postmark; or voter's registration card.) NO P. O. Boxes!)

For a Child Card (13 years and younger):

Complete and sign an application:

*Child must be able to write his/her first and last name.

*Application must include parent/guardian signature.

*Valid ID must presented at this time.
